In accordance with Texas law, clients have the right to access their health records, contact the appropriate licensing authority, and file a consumer complaint if needed. The information below explains how to do so.
Requesting Your Health Records
Clients may request access to their therapy records by submitting a written request. Requests can be made by emailing or contacting the practice directly. Please include your full name, date of birth, and the specific records you are requesting. Requests will be processed in accordance with applicable state and federal laws.
